Parkinson Network of Mt. Diablo Membership

We are pleased to have new and returning members in the organization. The $50.00 annual dues are a donation that covers the person with Parkinson’s and the spouse or caregiver. Dues help to defray our expenses, but they are not required to participate. Fill out the form below to let us know a little more about you and how we might be of service. Dues can be paid with your credit card or PayPal account by clicking on the PayPal link below the membership form.
